A member of our business office will be available to address your concerns and answer your
questions about insurance coverage and your financial responsibilities to the Center. You will
receive separate bills from Miracle Hills Surgery Center, anesthesia, laboratory/pathology and your surgeon.
It is the patient's responsibility to contact their insurance company for pre-approval of their
procedure - see the back of the insurance card for the telephone number to call.
INSURANCE: As a courtesy to you, our patient, we will file a claim with your insurance carrier.
However, your insurance coverage is an agreement between you and your insurer. It is your
responsibility to remit payment for charges not covered by your insurance. If a problem occurs
with your claim, you may be asked to assist in the resolution with your insurance carrier. If your
insurance plan requires you to pay a predetermined co-payment amount, that amount is due at
the time of admission.
The balance of your account is required to be paid in full within three months of Miracle Hills Surgery Center
receiving payment from you insurance carrier. Your account must have payment every thirty (30) days.
We accept Cash, Personal Checks, Visa, MasterCard, Discover, and CareCredit.
